In each of the following situations, an assessor believes a noncompliance could possibly have arisen with regard to the QA standard.

Example:	A team of assessors assessing an organisation on a two day assessment determine that there was a general lack of documented QA working instructions in the majority of departments including: purchasing, sales contracts, design and manufacturing.

When visiting the office of a large organisation while conducting an assessment, an assessor discovered that there was no method used for recording customer complaints in accordance with the procedure he had seen during his desk study.

3.	The assessor investigating an aviation engine assembly line came across an operative using a torque wrench that wasn't labelled as being part of the calibration system.  After discussing this with the shop foreman, he was told that the torque wrench was brand new that very day and new instruments don't need calibration certificates. 審核員在審查中發現某航空引擎流水線操作在運用力矩扳手，但其並未規定作為校準體系之一，經與車間主任討論，被告之那是新的品牌，其新儀器不需要驗證的

4.	The assessor investigating a training organisation found when talking to staff that they often signed on students for evening courses on the basis of numbers needed to run the course.  The lecturer then took their money, reported to the registry and obtained their student registration for them as if they had officially enrolled on registration day with no reference to their background experience. 審核員審查公司之培訓發現，跟員工交談，他們通常所指參加晚上培訓的並依據人數而開辦的課程。此類講座既花資金，同時要為報名人員到註冊處註冊，就象在官方招收有關審核人員，無其他背景和經驗來參考

5.	An assessment of an organisation providing servicing operations at client sites showed that weekly site visit reports were not always being completed by Area Supervisors.  On two sites, the Service Engineers said that they had not seen the Supervisor for a few weeks. 對公司在顧客處提供服務運作評估發現每周有監督員未完成該地區報告。在其他兩處，服務處工程師言，他們已經有幾星期沒有看到報告了
